	Please make sure forms are turned in to the proper persons. 2009-2010 Marching Yellow Jackets Registration Form - Due April 30, 2009 along with $50.00 deposit ($80.00 if going to Cedar Point) Enrichment Credit Form - Due by the 8th week of each quarter. Please indicate which activities were completed and be sure to attach any accompanying documents if requested. Handbook Agreement Form - Page 18 of handbook. Print, sign and turn in the first day of band camp for marchers, first day of school for non-marchers. Lincoln Center Ticket Order Form - Complete requested information and mail it, along with payment, to the address on bottom of the form before February 28. For faster service, you may also fax a credit card order to 480-654-2553. Lock-In Participation Form - Due the Tuesday before the Lock-In Medical Release Form - Due the first day of Band Camp in August, or first day of school for non-marchers. Student Account Withdrawal Form - Used to withdraw money from individual student accounts to help pay for items such as trips, fees, and uniform expenses. Student Information Form (Non-marcher) - Due the first day of school for all students not in marching band. Trailer Bee Order Form - Complete and mail with payment to the P.O. Box address listed on the form. Uniform Request Order Form - Can be used to order uniform replacement items such as new tux shirts, shoes, bow ties, etc. Uniform Rental Agreement Form - Due at Uniform Issue Day in August Yard Sign Order Form - Complete and mail with payment to the P.O.
